Abstract: 

Brain tumors in elderly are increasing as the number of people, who comprise the older population, does. About half of the patients with brain tumors appear to be over 60 years of age. In this review article, Glioblastoma multiform, as the most common malignant tumor of the central nervous system (CNS) in elderly, is discussed in details of definition, prognosis, diagnosis, treatment and differential diagnosis. Other tumors such as meningioma, pituitary adenoma, the CNS lymphoma and metastasis are also included to be reviewed. Treatment plans, either conservative or aggressive, classic or novel, approved or under investigation, are presented. Furthermore different attitudes of treatment in the past and recently are also argued. Conventional therapy, surgery, radiotherapy, chemotherapy radioimmunotherapy, hormonal therapy and some other novel methods of treatments are discussed in details for the glioma. Determining factors which may be associated to the patient’s response to each treatment plan are also discussed. Finally, some age related issues are provided to be paid attention to consider an old patient with brain tumor, and planning an optimal treatment in order to make the best management decisions. Until recently, people with brain tumors in elderly, were used to be treated in conservative plans and often were excluded of the clinical trials but now the number of patients who desire and receive more aggressive therapy for brain tumors is increasing.

Today the modality of choice for brain tumors is MRI with and without GD.GD injection needs during to stage T1, before injection and after injection for image subtracts to see the enhancement degree, detecting crystallized calcification, colloid and fat material also methemoglubin inside the tumor.- In case of thin layer seeding, GD MRI could be positive but T2 and FLAIR images could be negative "no mass effect".- For meningiomas if you do not want to inject contrast media please request plain CT SCAN that is stronger than non-contrasted MRI but GD MRI is the choice and better than both.- For small or micro-vestibular schwannoma do not request CT please request MRI with GD.- In craniopharyngioma request non-contrast CT with combination of MRI with and without GD.- For micro-adenoma request dynamic MRI "better than Dynamic CT"- please do not use axial CT and non-contrasted coronal CT for micro-adenoma".- Few infiltrative non-enhancing tumors need serial MRI to be differentiated from CVA.- For differentiation of tumor recurrency from radiotherapy necrosis MRS is necessary.Other lecture notes will be discussed in the round table.

Abstract: 

Telomere is a special structure protects the ends of human chromosomes to degrade. Telomere becomes shorter after each cycle of cell division. However, telomerase is an enzyme activated in cancer cells and germ lines rebuild telomere. However, apparently there is no correlation between telomere length and telomerase activity. The enzymatic activity of telomerase correlates with tumor malignancy. In general, telomerase activity and histological grade are too closely correlated and the results of studies indicate that telomerase activity may be an important malignancy marker in brain tumors. As a conclusion, we can say that activity of telomerase should be considered as a prognostic biological marker of response to treatment.

Abstract: 

Studies about intraoperative cytodiagnosis of central nervous system tumors by crush cytologic preparation have yielded favorable results.Crush preparation technique was applied to 77 fresh central nervous system specimens for immediate cytologic evaluation. This method is simple and particularly useful when the biopsy material is minuscule.The results of this technique were compared with formalin-fixed biopsy report and the accuracy was about 92.2 percent.

Abstract: 

Background and Aim: Early diagnosis of brain tumors according to ocular symptoms is important considering the fact that sometimes ocular finding is the first presentation. The purpose of the present study was to investigate ocular findings in patients with brain tumors. Materials and Methods: In this retrospective study 100 patients with documented brain tumors that were operated on in Ghaem Hospital, Neurosurgery Department, were reviewed and the ocular findings were analyzed statistically. Results: Out of 100 patients, 41 were male and 59 were female with the mean age of 32 years. Bilateral papilledema in 56%, unilateral papilledema in 9%, visual field effect in 14%, unilateral optic disc atrophy in 15%, petosis in 2%, proptosis in 3%, six nerve palsy in 12% and third nerve palsy in 3% of cases were found. Conclusion: Due to the fact that ocular findings are sometimes the first symptom to occur in patients with brain tumors and these patients may refer to an ophthalmologist or optometrist so early diagnosis and referral to a neurosurgeon can be helpful.
